Epiphany’s Spa Agreement

The purpose of this agreement is to confirm the services to be rendered for the spa services along with the name(s) of those responsible for payment of services provided by Epiphany Salon. Once this agreement has been filled out and signed the services scheduled. Epiphany Salon requires seven calendar days notice of any cancellation(s) of appointments listed on this contract. If, for any reason any member of the party does not show up for a scheduled appointment(s), and does not give seven calendar days notice of cancellation, the party responsible for this contract will be held responsible for the payment in full of any such service(s).

This agreement must be read, signed and filled out before/at the time of scheduling of services.

Helpful Hints and Suggestions

Attire:
Wear comfortable clothes so they are easy to remove for multiple spa services.  You always want to be relaxed and comfortable during your day.

What to Bring:
Bring a hair tie, brush, and bag to be able to keep some of your belongings with you. Also a snack if your package does not include a lunch.

Enjoy:
This is your special day, make it all about you. Epiphany Salon provides coffee and tea for your refreshment; however, please feel free to bring in any food, catering or beverages that you would like to have.

Northern Express:

• 3 hours of relaxation designed for today’s busy lifestyle. Indulge and de-stress. Our Georgia maids are fully dedicated to achieving perfection.
• Services include: Caribbean Manicure, Caribbean Pedicure, and Shampoo Style with a sensory experience.
• Finish with a makeup touch up.

Restoration Package:

• Designed to calm and restore your mind, body and soul.
• 4 hours include: an Elemental Nature Facial OR 60 min. Body Message,
• Caribbean Manicure, Caribbean Pedicure, Shampoo Style with a sensory experience.
• Finish with a makeup touch up.

The Ultimate Retreat:

• An all day spa retreat that starts with an Elemental Nature Facial, Full Body Massage, and Pedicure. Enjoy a provided lunch. Followed by a Manicure, Shampoo Style with a sensory experience.
• Finish with a makeup touch up.

Just for Him:
An all day spa retreat that starts with an Elemental Nature Facial OR Back Facial, Full Body Massage, and a Caribbean Pedicure. Enjoy a provided lunch. Followed by a Manicure, Shampoo with Hair Cut and finished style.
